EU AI Act Annex IV Technical Documentation Template

Annex IV is the single most labour-intensive deliverable in the AI Act: the technical dossier a provider of a high-risk AI system must compile before placing it on the EU market. We've structured it as a fillable template engineers can populate in days instead of weeks.

  5. Detailed description of the risk management system — Art. 9 chain, identified + foreseeable risks, mitigation per lifecycle stage.
  6. Description of any change made through pre-determined parameters — continuous learning bounds.
  7. Standards and specifications applied — harmonised standards (when published), common specs.
  8. Copy of the EU declaration of conformity.
  9. Description of the post-market monitoring plan — Art. 72 obligation operationalised.
